Sclerosing lipogranuloma of penis: Case report

Introduction: Increasing the volume of penis by injecting various substances (paraffin, silicone, mineral oils) remains a highly controversial procedure. This procedure is usually performed patients themselves or persons without licensed medical education. Injecting these causes local inflammatory reaction with redness, pain, swelling, thickening penile skin, subsequent formation sclerosing lipogranuloma, In more serious cases ulceration, necrosis, embolism and even death may occur. Impotence, painful erections dysuria are most common symptoms. The treatment excision reconstruction, while conservative approach using antibiotics has limited effect. Case report: A 45-year-old patient presents to Emergency Department due lesions on penis. Inspection showedscarring ulcers signs inflammation ventral side penis, was referred urologist. Part scar tissue excised, biopsy taken, reconstruction made. Pathohistological analysis revealed fragments skin subcutaneous connective tissue. Ulcerations infiltrate, hyperkeratosis parakeratosis were present parts epidermis. Inflammatory infiltrate rich in epithelioid histiocytes lipid vacuoles tissue, smaller foci necrosis cystic changes form "Swiss cheese" observed. Based pathohistological findings clinical data, he diagnosed lipogranuloma. Conclusion: very dangerous. extreme this can result death, will never provide expected , satisfying results patients.
